Kidney salvage in a case of ruptured renal artery aneurysm: case report and literature review 1

Abstract

A 79-year-old woman presented with hypovolaemic shock caused by rupture of a left renal artery aneurysm. She was successfully treated by arterial reconstruction with functional salvage of the kidney. The frequency of renal artery aneurysms, their risk of rupture and the results of urgent surgery are discussed. © 1997 Published by Elsevier Science Ltd.

Case report

A 79-year-old woman was admitted to a county hospital with a history of sudden onset of abdominal pain in the left upper quadrant. On arrival the patient was in hypovolaemic shock with a blood pressure of 75/30 mmHg and a regular heart rate of 130 beats per min. The patient was initially stabilized with volume replacement. Physical examination revealed left upper quadrant tenderness with deep palpation but no signs of peritoneal irritation.

An aneurysm of the renal artery was first described in 1770 [7]. Until 1968 only 345 cases had been reported. According to Kment (cited in Ippolito and Le Veen [8]), renal artery aneurysms account for 1% of all [7] and 22% of visceral aneurysms [9]. Of some 240 000 autopsy studies the incidence of renal artery aneurysms was found to be in the range of 0.01–0.09% (Table 1). In one prospective post-mortem angiographic series renal artery aneurysm was found in 15 out of 154 (9.7%) unselected cases
