Traitement des crises
La soudaineté des crises, leur relative courte durée, classiquement 15 à 180 minutes (International Headache Society), nécessite d’utiliser des substances thérapeutiques à action rapide. Le traitement préventif des crises
De nombreux produits sont utilisables, ce qui témoigne de leur efficacité modérée. Avant de mettre en route un traitement de prévention, on tiendra compte du nombre de crises par 24 heures, de l’efficacité du traitement de celles-ci, de la durée de la période des crises.
Parmi les principaux problèmes en cas d’algie vasculaire de la face épisodique à période douloureuse de durée supérieure à un mois ou surtout, en cas d’algie vasculaire de la face chronique, on note le risque vasculaire
Stratégie thérapeutique
La stratégie thérapeutique du traitement de la crise est claire. Elle repose sur le sumatriptan par voie sous-cutanée ou l’inhalation d’oxygène.
En revanche, celle du traitement préventif varie selon le caractère épisodique ou chronique de l’algie vasculaire de la face. La corticothérapie, le vérapamil, le méthysergide sont les traitements les plus utiles de la forme épisodique et, dans la forme chronique, le carbonate de lithium.
