Introduction
Le mésothéliome pleural malin (MPM) est une tumeur rare et agressive développée au niveau des surfaces mésothéliales de la cavité pleurale. Les séreuses péritonéale et péricardique (exceptionnellement la vaginale testiculaire) peuvent également être atteintes par le mésothéliome malin. Une exposition antérieure à l’amiante représente le principal facteur de risque du MPM. La prise en charge thérapeutique est difficile, parfois controversée, le MPM étant un cancer de mauvais pronostic, sans traitement curatif à ce jour. Dans cet article, sont rapportés les principaux éléments épidémiologiques, diagnostiques et thérapeutiques actuels du MPM.

Épidémiologie et facteurs de risques
En Europe, l’incidence du MPM est estimée à environ 20 cas pour un million d’habitants avec de grandes variations selon les pays. Le pic d’incidence de la maladie y est attendu entre 2015 et 2020, mais pourrait déjà être atteint dans certains pays, comme aux États-Unis. L’incidence est nettement plus élevée chez l’homme que chez la femme même si on observe une augmentation proportionnellement des cas féminins actuellement. Dépistage
L’intérêt du dépistage est de diagnostiquer de manière précoce une pathologie chez un sujet asymptomatique afin de lui proposer un traitement efficace. Sur la base des données disponibles sur le MPM (incidence, pronostic, thérapeutique) et des performances des outils de dépistage envisagés (imagerie, marqueurs biologiques…), l’intérêt du dépistage du MPM n’est pas démontré à ce jour [4]. Présentation clinique
Dans la majorité des cas, des signes cliniques non spécifiques sont présents. Il s’agit d’une toux irritative, d’une dyspnée et/ou d’une douleur thoracique, trouvés chez 60 % des patients [15]. Malgré une recherche importante, le seul traitement validé reste la chimiothérapie de première ligne. Si de nombreuses molécules ont été testées, seules quelques-unes ont démontré une efficacité suffisante pour motiver la réalisation d’essais de phase III. Le MPM reste une pathologie rare de prise en charge difficile et spécialisée, justifiant la mise en place actuelle, sous l’impulsion de l’INCa, d’un réseau de centres experts national et (inter)régionaux pour le MPM.
