Gonzalez-Alegre et al10 described the possibility of a genetic predisposition in some patients with OMD and even associated it with people who are good at public speaking, such as teachers, preachers, auctioneers, etc. Different treatments exist, but ObT-A treatment has been shown to be the most effective.9-16 OMD can reach such a level of severity that it can cause malnutrition problems.

Several case reports have been published of the treatment of patients with Lesch-Nyhan syndrome with associated self-injury resulting in oral wounds using BTX-A injections. Oromandibular dystonias can be seen in the oral and maxillofacial area as involuntary movements of the masticator, perioral, and tongue muscles.19 In the present case, our patient's neurodegenerative disorder (MLC) led to self-mutilating behavior of chronic lip biting, which resulted in the nonhealing ulcer.

Oromandibular dystonia consists of involuntary spasms of masticatory, lingual, perioral, and pharyngeal muscles, and may be classified as jaw opening, jaw closing, jaw deviation, or tongue protrusion type, or a combination.1 If blepharospasm is also present then the disorder is referred to as Meige syndrome.1,3 Botox is a potent neuromuscular paralysing agent, which causes reversible chemodenervation by blocking the presynaptic release of acetylcholine.4
