Zusammenfassung
Die Gesäßmodellierung mit Eigenfett, das sog. „Brazilian butt lift“, erregte in letzter Zeit große Aufmerksamkeit durch letale Fettemboliekomplikationen. Allen Fällen ist der Zusammenhang zwischen Eigenfettinjektion in die Glutealmuskulatur und nachfolgender Fettembolie gemein, was in der Literatur bereits detailliert untersucht wurde. Wir berichten hier über unsere Erfahrung mit diesem Eingriff in 901 Fällen mittels „360° superwet – Separation, Aspiration and Fat Equalization-Lipoaspiration“ (SAFE-Lipo) und „Expansion Vibrations Lipofilling“ (EVL) mittels streng subkutaner Fettinjektion in Bauchlage bei 30° Hüftgelenkflexion und konsequenter Ausrichtung der stumpfen 4‑mm-Exploding-Tip-Infiltrationskanüle, weg vom Muskel. Die durchschnittlichen Fettaspirationsmengen lagen bei 3600 cm3, die transplantierte Menge im Durchschnitt bei 1400 cm3 pro Gesäßhälfte. Eine erhebliche Verbesserung der „waist-to-hip-ratio“ und Optimierung der Körperproportion konnte mit dieser Technik ohne letales Risiko stets erreicht werden.
Abstract
Buttock enhancement surgery with autologous fat, the so-called Brazilian butt lift, has recently aroused great attention due to the fatal complication of fat embolism. All cases in common is the combination of autologous fat injection into the gluteal musculature and subsequent fat embolism, which has already been described in detail in literature. This article reports on the authors’ experience with this procedure in 901 cases using the “360° superwet – Separation, Aspiration and Fat Equalization Lipoaspiration (SAFE-Lipo)‑” and “Expansion Vibration Lipofilling(EVL)-technique” with strict subcutaneous fat injection in a prone position, 30° hip joint flexion and consistent alignment of the blunt, 4 mm exploding tip infiltration cannula away from the muscle. The average amount of fat aspirated was 3600 ml and the average transplanted amount was 1400 ml for each half of the buttocks. A substantial improvement of the “waist to hip”-ratio and optimization of the body proportions could always be achieved with this technique without case fatality rate.
