GERICO: 10 years of clinical research in geriatric oncology

Abstract

Clinical research in geriatric oncology is under active development, since aging is heterogeneous and requires specific programs addressing both old and new therapeutic strategies in elderly cancer patients. The group of geriatric oncology, GERICO, launched in 2002 by the French National Federation of Cancer Centres (FNCLCC), has acquired and demonstrated this specific experience. Convinced of the need to integrate the dimensions of functional status and quality of life — key issues in the elderly — it has built several trials on primary endpoints derived from geriatric assessment, different from the traditional tumour response and survival. Today it plans to integrate again classical criteria with geriatric ones. It militates in favour of inclusion of a lower selection of populations and takes into consideration life expectancy which remains so difficult to estimate.

Résumé

La recherche clinique en oncogériatrie est en plein développement, car l’hétérogénéité du vieillissement justifie le développement de programmes spécifiques aux sujets âgés pour évaluer les stratégies thérapeutiques classiques tout aussi bien qu’innovantes. Le groupe d’oncogériatrie GERICO de la Fédération nationale des centres de lutte contre le cancer (FNCLCC) lancé en 2002 illustre bien cemouvement. Convaincu de devoir intégrer les dimensions d’autonomie et de qualité de vie si importantes dans la population âgée, il s’est attelé à décliner des essais bâtis sur des critères de jugement tirés de l’évaluation gériatrique, différents des classiques critères carcinologiques de réponse et de survie. Aujourd’hui, il s’oriente vers une réintégration des critères carcinologiques traditionnels associés aux critères gériatriques. Il milite pour une moindre sélection des populations incluses et une prise en compte de l’espérance de vie si difficile à estimer.
