Abstract
Milestones in the development of bone substitutes are discussed with a short review of the history; bovine and coralliform calciumphosphates are compared with foamed synthetically manufactured materials. The first synthetically manufactured trabecular bone is compared with biological and synthetic materials. Such important parameters as biodegradation, osteoinduction, and osteoconduction are discussed, particular interest attaching to biostructure and osteomechanics. The demands on the materials vary with the different indications for their use, and a range of different materials therefore has to be available for optimal results to be achieved. At least four different bone substitutes are recommended for different purposes. The bone substitute of choice for tissue engineering should have the structure of a replica of the bone marrow spaces. All the materials have been subjected to histological evaluation.
