Zusammenfassung
Die anterior öffnende Osteotomie der proximalen Tibia ist ein seit Langem etabliertes Verfahren in der Behandlung des symptomatischen Genu recurvatum. Die Ursachen sind vielfältig und liegen oft im neuroorthopädischen Bereich oder entstehen posttraumatisch. Es gibt kapsuloligamentäre, ossäre und kombinierte Ursachen des Genu recurvatum. Die knöcherne Anatomie des Tibiaplateaus hat bei Verletzungen der Kreuzbänder zunehmend an Bedeutung gewonnen. Besonders bei Verletzungen des hinteren Kreuzbands (HKB) hat der tibiale Slope einen wesentlichen Einfluss auf das postoperative Ergebnis und die Entscheidungsfindung für eine konservative Therapie. Es ist bekannt, dass ein flacher Slope (<8°) für schlechte Ergebnisse nach HKB-Ersatz verantwortlich sein kann. Eine anterior öffnende Flexionsosteotomie stellt hier eine gute Behandlungsoption dar, um einen flachen Slope bei der HKB-Insuffizienz auszugleichen.
Abstract
Anterior open wedge osteotomy is a long-established procedure in the treatment of symptomatic genu recurvatum. The causes are manifold and often lie in the field of neuro-orthopedics or in a posttraumatic setting. There are capsuloligamentous, osseous and combined causes of genu recurvatum. The bony anatomy of the proximal tibia has gained in importance, particularly in injuries to the cruciate ligaments. Especially, in the treatment of injuries of the posterior cruciate ligament (PCL) the tibial slope has an essential influence on the postoperative result and decision making for conservative treatment. It is known that a flat slope (<8°) can be responsible for poor results after PCL reconstruction. An anterior opening wedge osteotomy represents an excellent treatment option to compensate for a flat slope in cases of PCL insufficiency.
