Zusammenfassung
Bei dem seltenen Fall von bilateralen Läsionen der Opercula frontoparietalia resultiert ein Symptomenkomplex aus Anarthrie, schwerster Dysphagie, Unfähigkeit des Kauens und unter Umständen einer zusätzlichen fazialen Parese. Gleichzeitig besteht eine willkürlich-automatische Dissoziation, d. h. die betroffene Muskulatur ist im Rahmen von unwillkürlichen Bewegungen funktionstüchtig. Dieses Syndrom ist unter den Synonymen Foix-Chavany-Marie-Syndrom (FCMS), (bilaterales) anteriores Operkulumsyndrom und fazio-pharyngo-glosso-mastikatorische Diplegie bekannt. Wir berichten von einem Patienten, der ein FCMS nach bilateralen Mediainfarkten entwickelte, die in einem Abstand von vier Jahren auftraten.
Summary
Bilateral lesions of the opercula frontoparietalia are uncommon and cause a symptom cluster including anarthria, severe dysphagia, inability to chew and sometimes facial paresis. At the same time there is an automatic-voluntary dissociation, meaning that the affected muscles are functional within the scope of involuntary movements. This syndrome is known as Foix-Chavany-Marie syndrome (FCMS), (bilateral) anterior operculum syndrome or facio-pharyngo-glosso-masticatory diplegia. We report the case of a patient who suffered from FCMS after having infarctions in the territory of the middle cerebral artery on each side 4 years apart.
