Summary
Calciumphosphate-ceramics have been intensively investigated as bone replacement materials for more than twenty years. In consequence to numerous experimental as well as clinical implantations Tricalciumphosphate-ceramic and Hydroxyapatite-ceramic proved to be the most suitable substances. Nevertheless, due to several shortcomings, these synthetic bone replacement materials can up to now in no way replace autogenous bone, the ’golden standard', for all indications of the clinical setting. The reason for this is that despite our ever increasing knowledge of materials' and host bed properties we still are unable to securely predict the outcome after the implantation of such synthetic substitutes. The article reviews the most important biological reactions in mammals and explains the underlying causes.
Zusammenfassung
Kalziumphosphatkeramiken unterschiedlicher Zusammensetzung werden seit nahezu 20 Jahren für den Knochenersatz in der Unfallchirurgie und Orthopädie empfohlen und eingesetzt. Unter diesen Materialien haben sich, aufgrund zahlreicher tierexperimenteller Untersuchungen – inzwischen – einer Vielzahl von klinischen Implantationen, Trikalziumphosphatkeramik und Hydroxylapatitkeramik als die am besten geeigneten, synthetischen Ersatzstoffe herausgestellt. Dennoch sind diese Verbindungen nicht ideal. Bis heute können sie keinesfalls für alle die Indikationen, unter denen autogener Knochen, als der ‚Goldenen Standard’, transplantiert wird, eingesetzt werden. Ursache dafür ist, daß die biologischen Reaktionen des Säugetierorganismus auf Kalziumphosphatkeramiken, trotz unserer stetig zunehmenden Kenntnisse in bezug auf Material- und Lagereigenschaften, nicht ausreichend sicher vorausgesagt werden können, um Aussagen über das zu erwartende Einwachsverhalten der Kalziumphosphatkeramiken auch in kritischen Situationen zu erlauben. Der Artikel beschreibt typische lokale Reaktionsformen in der Muskulatur und im Knochen und deren Ursachen.
