Endoscopic papillotomy, syn. sphincterotomy: Results from the past two years

  • Published:
Current Gastroenterology Reports Aims and scope Submit manuscript

Abstract

Endoscopic papillotomy, introduced in 1973, is now an established endoscopic procedure for treatment of various diseases of the papilla, bile duct, and pancreas. This article describes the use of this technique, the various instruments that can be employed, the instances in which it is indicated, and its associated complications and risks. Alternative treatments are also summarized.

This study deals with the important concern over post-ERCP and post-EPT pancreatitis and how to avoid it. In patients who bear a high risk of pancreatitis after this procedure, the authors were able to discern who would benefit from prophylactic insertion of a small-caliber stent into the pancreatic duct.

Pancreatic disruption and its various clinical consequences are described. Furthermore, the beneficial effect of stent insertion in the pancreatic duct is clearly demonstrated. These data confirm the results of earlier studies and are a strong stimulus for a more "aggressive" approach. The potential benefits for patients seem to outweigh the risks by far.
