Autologous transplantation and neurolysis of the brachial plexus following trauma

Summary

Autologous transplantation in brachial plexus injuries has become significantly more efficacious through increased knowledge of the microtopographic anatomy of the fascicle structures and the refinement of microneurosurgical techniques. Recovery of shoulder and upper arm functions can be achieved with autologous transplantation in the majority of patients under optimized conditions. However, attempts to restore forearm, hand and finger functions in this way have been disappointing so far. Therapeutic success is primarily governed by early decision making, selection of adequate surgical strategies and intensive and longlasting postoperative management.

Zusammenfassung

Die autologe Transplantation bei posttraumatischen Armplexusläsionen hat durch den Wissenszuwachs im Bereich der mikrotopographischen Nervenanatomie der Fazikelstrukturen und die Verfeinerung der mikroneurochirurgischen operativen Techniken entscheidend an therapeutischer Wirksamkeit und damit an Bedeutung gewonnen. Die Funktionsrückkehr im Bereich der Schulter und des Ellenbogens können unter optimierten Verhältnissen in der Mehrzahl der Patienten durch die autologe Transplantation erreicht werden. Wenig erfolgreich sind bislang die Versuche, Hand- und Fingerfunktion zu reaktivieren. Entscheidende Faktoren für den therapeutischen Erfolg sind die rechtzeitige Indikationsstellung, eine adäquate operative Therapie und eine sorgfältige und langfristige postoperative Betreuung der Patienten.
