La somiglianza chimica esistente tra acido urico ed allossana ha indotto l’A. ad eseguire determinazioni dell’uricemia in 314 soggetti diabetici. Solo un paziente era affetto da gotta, mentre la malattia non figurava nell’anamnesi familiare di nessuno di essi. Il valore medio dell’uricemia era di 4,8 mg/100 ml (SD ± 1,57) negli uomini e di 4,2 mg/100 ml (SD ± 1,59) nelle donne. Nei primi, i livelli uricemici erano significativamente più elevati che nelle seconde (p<0,001). In entrambi i sessi, essi aumentavano in maniera significativa con l’età. In 17 diabetici su 25 (68%) aventi livelli uricemici superiori a quelli «normali», i valori dell’azotemia superavano i 40 mg/100 ml. Résumé
La ressemblance chimique entre l’urate et l’alloxane suggéra de mésurer le niveau de l’urate dans le sérum de 314 sujets diabétiques. Un patient seulement était goutteux et personne ne possedait un précédent de goutte dans sa famille. Le niveau moyen de l’urate dans le sérum était de 4,8 mg/100 ml (SD ± 1,57) pour les hommes, et de 4,2 mg/100 ml (SD ± 1,59) pour les femmes. Les valeurs étaient significativement plus élévées pour les hommes que pour les femmes (p ≤ 0,001) et, dans les deux cas, elles augmentaient de façon significative avec l’âge des patients. Soixante-huit pour cents des diabétiques avec un niveau d’urate hors de la règle, c’est à dire 17 patients sur 25, possédait un niveau d’azotémie supérieur à 40 mg/100 ml. Cet étude ne confirme pas l’hypothèse d’une relation significative entre goutte, hyperuricémie et diabète.
Resumen
La semejanza química existente entre ácido úrico y aloxana ha inducido al autor a realizar determinaciones de la uricemia en 314 individuos diabéticos. Sólo un paciente estaba afectado de gota, mientras que la enfermedad no figuraba en la anamnesis familiar de ninguno de ellos. El valor medio de la uricemia era de 4,8 mg/100 ml (SD ± 1,57) en los hombres y de 4,2 mg/100 ml (SD ± 1,59) en las mujeres. En los primeros, los niveles uricémicos eran significativamente más elevados que en las segundas (p<0,001). En ambos sexos, aquéllos aumentaban con la edad de modo significativo. De 25 diabéticos, en 17 (68%) con niveles uricémicos superiores a los «normales», los valores de la azoemia superaban los 40 mg/100 ml. Los resultados del presente estudio no abonan la hipótesis de que entre gota, hiperuricemia y diabetes mellitus exista una relación significativa.
Zusammenfassung
Die Ähnlichkeit der chemischen Beschaffenheit von Harnsäure und Alloxan veranlasste den Verfasser bei 314 Diabetikern den Harnsäurespiegel des Blutes zu bestimmen. Nur ein Patient litt an Gicht, während bei keinem eine positive Familienanamnese für Gicht bestand. Der mittlere Harnsäurespiegel im Blut betrug 4,8 mg/100 ml (SD ± 1,57) bei den männlichen und 4,2 mg/100 ml (SD ± 1,59) bei den weiblichen Patienten. Die Werte waren bei den Männern wesentlich höher als bei den Frauen (p<0,001). Bei beiden Geschlechtern nahm die Harnsäurekonzentration im Blut mit dem Alter stark zu. Bei 17 von 25 (68%) Diabetikern mit über die «Norm» erhöhtem Harnsäurespiegel war auch der Blutstickstoff höher als 40 mg/100 ml. Aufgrund der Ergebnisse der vorliegenden Untersuchung erscheint die Hypothese, dass zwischen Gicht, Hyperurikämie und Diabetes mellitus eine bedeutsame Beziehung besteht, nicht gerechtfertigt.
Summary
The chemical similarity between urate and alloxan stimulated the measurement of serum urate in 314 diabetics. Only one patient had gout and none had a family history of gout. The mean serum urate level in males was 4.8 mg/100 ml (SD ± 1.57) whilst the mean value in females was 4.2 mg/100 ml (SD ± 1.59). Males had significantly higher levels than females (p<0.001). The levels in both sexes increased significantly with age. Seventeen of the 25 diabetics (68%) with serum urate levels above the ‘normal’ range were known to have blood urea levels of over 40 mg/100 ml. This study does not support the hypothesis of a significant relationship between gout, raised serum urate levels and diabetes mellitus.
