Serologische Reaktionen beim akuten und chronischen Gelenkrheumatismus und ihre klinische Bedeutung

Zusammenfassung

Die sog. Akute-Phase-Reaktionen, die Methoden zum Nachweis des Rheumafaktors und der Antikörper gegen hämolytische Streptokokken werden besprochen. Die Akute-Phase-Reaktionen gestatten eine zuverlässige Aussage über die Aktivität und durch die serologische Verlaufsbeobachtung auch eine begrenzte Aussage über die Prognose rheumatischer Erkrankungen; sie erbringen keine Hilfe für die Unterscheidung der akuten Polyarthritis von akuten Phasen der chronischen Polyarthritis. Die Methoden zum Nachweis des Rheumafaktors und der Streptokokken-Antikörper gestatten keine Aussage über die Aktivität und eine begrenzte Beurteilung der Prognose nur durch die serologische Verlaufsbeobachtung; sie sind dagegen ein wichtiges Hilfsmittel in der Differentialdiagnose. Alle mit den verschiedenen Methoden nachgewiesenen Serumfaktoren sind an der Entstehung der akuten und der chronischen Polyarthritis nicht primär ursächlich beteiligt. Die Pathogenese des akuten und des chronischen Gelenkrheumatismus bleibt auch mit den bisher vorliegenden serologischen Untersuchungen ungeklärt.
