Summary
Uric acid concentration and their relationship to different types of hyperlipoproteinemia were investigated in three groups of out-patients. Group A: 273 patients with primary hyperlipoproteinemia without any clinical signs of cardiovascular disease; Group B: 149 patients with a history of myocardial infarction; Group C: 40 patients with primary gout. The results were compared with data derived from randomized healthy persons (employees of the Free University of Berlin).
Patients of group A with hyperlipoproteinemia of type IV have higher uric acid levels than patients with type IIa, type IIb hyperlipoproteinemias and the normal controls. However, there was no difference between type IIa, type IIb and the normal controls. Patients in group B with hyperlipoproteinemia of type IV also show the highest uric acid levels. Patients with type IV have comparable serum uric acid levels within group A and B. There exists no correlation between levels of triglycerides and uric acid. In group C 67% of all patients had an hyperlipoproteinemia of type IV, the incidence of types IIa and IIb, however was the same as in the normal controls. These results clearly demonstrate the association of primary hyperlipoproteinemia of type IV and hyperuricemia and on the other hand the close relationship between primary gout and type IV hyperlipoproteinemia.
Compared with the normal controls type IV patients of group A with normal weight, normal blood pressure and normal glucosetolerance had higher uric acid levels. Consequently, the elevated uric acid levels in patients with primary hyperlipoproteinemia of type IV cannot be explained alone by concomitant factors such as obesity, arterial hypertension and glucose intolerance.
Zusammenfassung
Zusammenhänge zwischen Harnsäurekonzentrationen im Serum und verschiedenen Hyperlipoproteinämie-Typen wurden bei drei ambulanten Patientengruppen untersucht: Gruppe A: 273 Patienten mit primären Hyperlipoproteinämien ohne nachweisbare kardiovasculäre Erkrankungen, Gruppe B: 149 Patienten mit Zustand nach Myokardinfarkt, Gruppe C: 40 Patienten mit primärer Gicht. Als Vergleichskollektiv dienten Stichproben aus der Einstellungsuntersuchung des Personals der Freien Universität Berlin.
Die Patienten der Gruppe A mit Typ IV hatten höhere Harnsäurewerte als die Patienten mit Typ II a, II b und das Vergleichskollektiv. Dagegen bestanden keine Unterschiede zwischen Typ II a, II b und dem Vergleichskollektiv. Auch innerhalb der Gruppe B hatten die Patienten mit einer Hyperlipoproteinämie vom Muster des Typ IV die höchsten Harnsäurewerte. Die Patienten mit Typ IV der Gruppen A und B hatten vergleichbare Serumharnsäurekonzentrationen. Ein Zusammenhang zwischen der Höhe der Triglyzeridund Harnsäurespiegel konnte bei ihnen nicht nachgewiesen werden. In der Gruppe C hatten 67% der Patienten eine Hyperlipoproteinämie vom Typ IV. Die Typen II a und II b waren dagegen nicht häufiger als beim Vergleichskollektiv. Diese Befunde sprechen für eine enge Koppelung zwischen primärer Hyperlipoproteinämie vom Typ IV und hohen Harnsäurewerten einerseits sowie primärer Gicht und Hyperlipoproteinämie vom Typ IV andererseits.
Patienten mit Typ IV der Gruppe A mit Normalgewicht, normalen Blutdruckwerten und normaler Glucosetoleranz hatten gegenüber dem Vergleichskollektiv weiterhin höhere Harnsäurewerte. Die erhöhten Harnsäurewerte bei Patienten mit primärer Hyperlipoproteinämie vom Typ IV können somit durch das gehäufte Vorkommen von Adipositas, arterieller Hypertonie und Glucosetoleranzstörungen allein nicht erklärt werden.
