Zusammenfassung
Die Arzneimittelverordnungen von 603 im Herzinfarktregister des MONICA Projektes Augsburg registrierten Herzinfarktpatienten wurden mit Hilfe eines Arzneimittelinformationssystems (SMA) auf das Vorliegen von möglichen Arzneimittelwechselwirkungen geprüft. Vor dem Infarkt sind 59% der Patienten mit durchschnittlich 2 Wirkstoffen behandelt, bei der Entlassung aus dem Krankenhaus 100% mit 4,5 Wirkstoffen, und ein Jahr nach dem Infarkt 965 mit 5,8 Wirkstoffen. Vor dem Infarkt enthalten 18% der Verordnungen dieser Patienten mögliche Arzneimittelwechselwirkungen, bei Entlassung 65%, und ein Jahr, nach dem Infarkt 66%. Während die potentielle Häufigkeit und der Schweregrad der Wechselwirkungen eher geringgradig ist, enthalten Verordnungen häufig mehrere mögliche Wechselwirkungen. überschlägige, Berechnungen ergeben, dass mindestens 5–6% der Verordnungen bei Herzinfarktpatienten früher oder später Wechselwirkungen hervorrufen werden. Les prescriptions de médicaments de 603 patients enregistrés dans le Registre des infarctus du myocarde du projet MONICA Augsburg ont été examinées à l'aide d'un système d'information sur les médicaments (SMA), dans le but de détecter les interactions de médicaments. Avant l'infarctus, 59% des patients sont traités avec en moyenne 2 agents thérapeutiques, après leur sortie de l'hôpital: 100% avec 4,5 agents thérapeutiques, et un an après l'infarctus 96% avec 5,8 agents thérapeutiques. Avant l'infarctus, 18% des prescriptions comprennent d'éventuelles interactions d'agents thérapeutiques, aprés la sortie de l'hôpital 65% et un an après l'infarctus 66%. Alors que la fréquence potentielle et le degré de gravité des interactions sont plutôt minimes, les prescriptions comprennent souvent plusieurs interactions possibles. Des calculations approximatives montrent qu'au moins 5–6% des prescriptions provoqueront tôt ou, tard des interactions chez les patients. Les béta-bloquants constituent le groupe d'agents thérapeutiques donnant lieu le plus souvent à d'éventuelles interactions de médicaments.
Summary
The prescriptions of 603 patients who had survived acute myocardial infarction (AMI) for at least one year and were registered from 1 Oct 84 to 31 Dec 86 in the Coronary Event Register of the MONICA project Augsburg were analyzed, for the presence of drug-drug interactions with the aid of a computerized drug information system (SMA). Prior to AMI, 59% of patients were treated (average of 2 active substance per patient), 100% were treated on release from hospital (4.5 active substances), and 96% one year after AMI (5.8 active substances). Potential drug-drug interactions were found in 18% of patients before AMI, 65% on discharge from hospital and 66% one year after AMI. While the potential frequency and severity of interactions are minor on average, prescriptions frequently contain several interactions. Calculations show that at least 5–6% of all prescriptions after AMI will produce interactions so that drug safety for this high-risk patient group can be enhanced by a drug information system. Beta-blockers are the substance group most frequently involved in potential interactions in AMI patients.
