Approach to the Patient with Bundle Branch Block

Abstract

Block or delay of conduction in the branches of the His bundle is manifested by widening of the QRS on the electrocardiogram. A QRS width greater than or equal to 0.12 s in duration during antegrade conduction in supraventricular or junctional rhythm is arbitrarily defined as “complete” bundle branch block. This article will consider the approach to bundle branch block in two major areas of clinical interest:

  1. 1)

    bundle branch block in association with acute myocardial infarction, and

  2. 2)

    chronic bundle branch block in persons with and without symptoms of transient neurological impairment.
