Zusammenfassung
Aufgrund der zumeist Jahrzehnte langen Latenzzeit von der Asbestexposition als den auslösenden Hauptrisikofaktor bis zur eigentlichen Krankheitsmanifestation sind vorzugsweise ältere Patienten im 6.–8. Lebensjahrzehnt von einem Pleuramesotheliom betroffen. Dies erfordert in der klinischen Praxis altersadaptierte Verfahren von der Diagnostik, im Weiteren über die palliative Pleuraerguss-Entlastung mittels eingelegtem Pleura-Dauerkatheter oder mit Talkum-Pleurodese bis hin zu operativen Resektionen in Form der Pleurektomie und Dekortikation. Systemtherapeutisch steht seit 2004 der Multi-Target-Enzyminhibitor Pemetrexed als Antifolat im Vordergrund der therapeutischen Bemühungen in Kombination mit Platin-Derivaten oder als Monotherapeutikum. Bei Tumorrezidiven und/oder Progredienz kann abhängig von Allgemeinzustand und Patientenwunsch auf Gemcitabine oder Vinorelbin zurückgegriffen werden.
