Abstract

Die laparoskopische Cholezystektomie stellt den therapeutischen Goldstandard der Behandlung der symptomatischen Cholezystolithiasis dar und ist mit 175.000 Eingriffen pro Jahr eine der häufigsten Operationen in Deutschland. Über 90% aller Cholezystektomien erfolgen heute laparoskopisch. Trotz der steigenden operativen Erfahrung geht die laparoskopische Cholezystektoemie mit einer signifikanten Komplikationsrate einher, die nach wie vor höher als bei der „offenen“ Osteotomietechnik liegt. Verletzungen der Gallenwege, Gallenfisteln, Blutungen und Darmläsionen gehören zu den schweren Komplikationen der laparoskopischen Cholezystektomie. Diese Komplikationen entstehen meistens infolge einer zu weiten Indikationsstellung bzw. unzureichender Patientenselektion, mangelnder chirurgischen Erfahrung sowie der immanenten chirurgisch-technischen Einschränkungen der laparoskopischen Chirurgie.
