Zusammenfassung
Untersuchungen mit faradischer bzw. galvanischer Stimulation des motorischen Kortex haben wesentlich zum Verständnis der somatotopischen Organisation des motorischen Kortex beigetragen (Ferrier 1875; Foerster 1927; Penfield u. Boldrey 1937). Eine Anwendung direkter kortikaler Stimulation in der klinischen Neurophysiologie wurde aber erst möglich, nachdem Merton u. Morton (1980) einen elektrischen Stimulator vorstellten, der es ermöglichte, beim intakten Schädel transkraniell direkt den motorischen Kortex zu stimulieren. Eine breitere Anwendung dieser Methodik und insbesondere ihre Anwendung im Kindesalter war aber noch dadurch begrenzt, daß die Untersuchung relativ schmerzhaft war. Ein wesentlicher Durchbruch wurde Mitte der 80er Jahre durch die Entwicklung magnetoelektrischer Stimulatoren erzielt (Barker et al. 1985). Dabei wird durch Erzeugung eines kurzen gepulsten Magnetfeldes ohne störende schmerzhafte Irritationen der Haut, in einigen Zentimetern Tiefe, ein ausreichend hochenergetisches elektrisches Feld erzeugt, das ausreicht, den motorischen Kortex oder periphere Nervenabschnitte bzw. Nervenwurzeln zu stimulieren. Sowohl die elektrische wie die magnetoelektrische Stimulationsmethode sind seither bei Erwachsenen in klinischen Studien an Patienten mit Entmarkungserkrankungen (Cowan et al. 1984; Ingram u. Swash 1988), mit zerebrovaskulären Läsionen (Berardelli et al. 1987; Hömberg 1989; Hömberg et al. 1989), bei zervikalen Myelopathien (Abbruzzese et al. 1988) und anderen spinalen Läsionen (Thompson et al. 1987), bei degenerativen motoneuronalen Erkrankungen (Ingram u. Swash 1987) sowie bei extrapyramidal-motorischen Erkrankungen (Dick et al. 1985; Thompson et al. 1986; Hömberg u. Lange 1989) eingesetzt worden.
