Zusammenfassung

Wegen der zahlenmäßigen Bedeutung stellt das Bronchialkarzinom auch in der klinischen Radioonkologie eines der großen Probleme dar. Therapie der ersten Wahl ist im Fall des nichtkleinzelligen Bronchialkarzinoms die operative Resektion, beim kleinzelligen Karzinom die Chemotherapie. Obwohl die Strahlentherapie in frühen Stadien nur bei medizinischer Inoperabilität primär zum Einsatz kommt, spielt sie bei 1/3 aller betroffenen Patienten vorwiegend in den fortgeschrittenen Stadien eine maßgebliche Rolle. Indikationen sind beim nichtkleinzelligen Tumor die adjuvante Bestrahlung nach Resektion sowie die hohe Anzahl aufgrund des Stadiums oder aus funktionellen Gründen inoperabler Fälle, beim kleinzelligen Karzinom die konsolidierende Bestrahlung des Mediastinums sowie die elektive Bestrahlung des Hirnschädels. Darüber hinaus hat sich die Strahlentherapie als effiziente Maßnahme zur Palliativbehandlung von Hirn- und Knochenmetastasen erwiesen.
