The International System for Serous Fluid Cytopathology

Abstract

Excess fluid in serous cavities is an abnormal state and quite variable; thus, standards for specimen adequacy are difficult to establish. This chapter discusses the current status of serous fluid adequacy and provides guidance for ensuring discovery of pathologic entities. There are no large-scale studies to support the use of cell numbers for adequacy. An adequate sample should be representative of the underlying disease process, in the context of clinical and radiologic findings. As the disease processes leading to effusions may vary widely in their cellularity and composition, the absence of mesothelial cells does not always necessitate categorization as non-diagnostic (ND). Generally, this category should be uncommon in serous fluid reporting.
