Medial Open-Wedge High Tibial Osteotomy

Abstract

Medial open-wedge high tibial osteotomy is indicated for constitutional and post-traumatic varus malalignment of the tibia. We describe a biplanar open-wedge technique performed by a limited medial incision. A detailed preoperative deformity analysis and preplanning are mandatory. The surgical technique includes an oblique incomplete osteotomy of the posterior two-thirds of the tibia performed with an oscillating saw and guided by two K-wires. A second osteotomy is performed 100° to the first cut in the transversal plane behind the tibial tuberosity. The osteotomy gap is gradually opened with chisels and an osteotomy spreader. An alignment rod was developed to allow for intraoperative verification of the correction. Fixation is achieved with a specific plate fixator which is inserted percutaneously (MIPO). The technique can also include corrections in the sagittal plane (slope). Details of the surgical technique, rehabilitation, and management of complications are discussed.
