How to Manage Recurrent Craniopharyngiomas

Abstract

Craniopharyngiomas are benign disembryogenetic tumors accounting for the 2–5% of all intracranial tumors and they present high rates of recurrence even after radical resection and surgical treatment of these latter cases is more challenging.

Several transcranial approaches have been described as possible surgical options for the treatment of craniopharyngiomas. The endonasal technique offers a direct approach that permits access to the suprasellar, retrosellar, and retroclival space, obviating brain retraction. Based on our experience with this surgical technique, we herein details variations required to treat recurrent lesions, depending on the original surgical treatment: transcranial, transsphenoidal (standard microscopic or endoscopic) expanded endonasal approach, and the eventual alternative treatments.
