Preventing Pancreatic Fistula Following Distal Pancreatectomy

Abstract

Pancreatic fistula is a common and highly morbid complication of distal pancreatectomy. Patient factors and technical factors appear to contribute to the incidence of postoperative pancreatic fistula (POPF). Numerous clinical trials and retrospective studies have investigated many of these with an aim toward identifying modifiable risk factors that could be controlled. Data on most of these risk factors remain relatively unconvincing. Careful surgical technique, ligation of the main duct, where technically feasible, and use of a mesh may be beneficial interventions. Octreotide and pancreatic duct stenting may have use in highly selected patient groups.
