Abstract

In this chapter, data on human material only are reviewed, since it is not possible to describe animal studies due to limitations in the scope of this volume. Only where there is a lack of human data have I briefly discussed animal experiments in that particular area. Space restrictions have forced me to omit from the finished review some of the text and some references. As a result, some comparative aspects have disappeared, and the priority of the researchers is not always apparent. The originators of an approach are then only cited in the listed work by their followers. I therefore apologize to the authors in question and to the reader for this incompleteness.
